Daisuke Matsuzaka struggled in a rehab start for the Portland Sea Dogs this afternoon, going two innings and giving up five runs on four hits to the New Hampshire Fisher Cats in Manchester, N.H. Matsuzaka had three walks and one strikeout on the day and threw 58 pitches.
The first inning was abysmal for Dice-K. The 28 year-old righthander gave up a leadoff home run to Todd Donovan and needed 49 pitches to get out of the first. He allowed five runs on four hits and three walks in the inning. Matsuzaka retired the side on nine pitches in the second inning.

It seemed that even when he was going reasonably well, Matsuzaka would from time to time have these 'blow up' innings where he forgot how to pitch a baseball.

I don't think that part of his game has anything to do with shoulder fatigue.
